If you’re not sure whether you have out-of-network reimbursement, I’d recommend calling your insurance company and asking the following questions:
Do I have out-of-network coverage?
If so, what is my deductible?
Is my plan on the calendar year, fiscal year, or “plan year”?
What is the percentage covered of each session (CPT code 90834)?
What is the usual and customary rate (in other words, how much does the insurance company consider to be the going-rate per session)?
