I am in-network for Consolidated Health Plan (CHP). I do not accept any other insurance plans.

If you have out-of-network benefits through your insurance plan, I’m happy to complete the paperwork for reimbursement. If you’re not sure whether you have out-of-network reimbursement, I’d recommend calling your insurance company and asking the following questions:

  • Do I have out-of-network coverage?

  • If so, what is my deductible?

  • Is my plan on the calendar year, fiscal year, “plan year”?

  • What is the percentage covered of each session (CPT code 90834)?

  • What is the usual and customary rate (in other words, how much does the insurance company consider to be the going-rate per session)?
